How to administer the kit: 

1. The video shows how a patient could self administer a kit
2. ADVENT team members will do the finger prick portion for patients
3. ADVENT team members will drip the blood onto the card for patients

For safety:

1. ADVENT team members will wash hands, wear gloves
2. ADVENT Team members will throw away skin lancet, any bloody materials 

Register the kit:

1. ADVENT team members will register the kits at wyndly.com/test. The patient will complete the first part of the registration and then pass the iPad to the team member.
2. ADVENT team members will print a face sheet and include it with the Sample
3. ADVENT team members will follow clinic specific instructions on where to place the sample for pickup to lab.

Anyone with allergy symptoms such as:

-Runny nose
-Watery eyes
-Itchy eyes or nose
-Sneezing
-Allergies to cats or dogs

But also often recurrent sinusitis or turbinate hypertrophy are associated with allergic rhinitis. By testing for and treating allergies, we can ensure the gains Advent’s team makes with surgery are maintained in the long term.

We tell patients at our office that we have a simple method for us to check their allergies:

-Instead of a multiple stick, painful skin test, we can now quickly do a single finger prick blood test to check for allergies today in the office.
-The lab will bill your insurance, and we've negotiated an out-of-pocket maximum that is still cheaper than most patients pay in copays for skin prick testing.
-We will get the results back in a few weeks and can prescribe therapy based on those results if needed.

Sublingual immunotherapy works by exposing your body to small doses of what you’re allergic to, and over time, your body becomes desensitized. This helps preserve the benefits of nasal surgery and avoids allergies causing inflammation in the nose later on. It also lets you stop using as many antihistamines and nasal sprays.

It uses the same treatments as allergy shots, but instead of a weekly shot in the office for 3-5 years, you can take sublingual immunotherapy under the tongue at home without any shots.

Most patients begin to feel better within 3-6 months whereas allergy shots can take up to a year. And studies show it has similar outcomes without the pain of shots, and it’s significantly safer which is why you can take it at home. 

Because of this convenience, it isn’t typically covered by insurance, but some patients can use their FSA or HSA and even out of pocket, it’s typically cheaper than your copays for weekly shots for 5 years.

According to the AAO-HNS Clinical practice guideline on immunotherapy for Inhalent Allergy the patients who should not get SLIT are:

-SLIT should not be started during pregnancy. It can be continued on a patient on therapy who becomes pregnant but should not be started in a pregnant patient

-Patients with uncontrolled or severe asthma (patients who are having unexpected or frequent attacks and are not on therapy or unable to control their symptoms using therapy or patients who have to go to the ER for asthma attacks).

-Patients who are unable to tolerate an Epi pen (which is used in case of a severe reaction)

-Patients with eosinophilic esophagitis

Who is a less ideal candidate:

-Patients who are immunosuppressed-Patients with a history of anaphylaxis

In 2003, and again in 2010, the Cochrane Review, an independent organization for evidence based treatment, reviewed 314 of scientific studies and concluded that sublingual immunotherapy is both effective and safe for treatment of environmental allergies.

Yes! Because SLIT uses the same treatments as allergy shots, patients are able to easily transition between shots and SLIT. SImply provide your provider with what you are being currently treated for using shots or undergo new testing. SLIT is great for busy parents, travelers, or just people who can’t do weekly visits to an office.

Fixing a patient’s allergies can lower inflammation of their nasal lining and improve their long-term breathing and sleep outcomes.

Wyndly's allergy test report makes it easy to understand a patient’s allergy profile.

1. The triggers a patient reacted to are clearly listed as well as the severity.
2. Always confirm with the patient whether these triggers correlate with their symptoms.
3. Each report will come with a suggested SLIT Rx which Wyndly’s team will formulate per the patient’s results. Simply review the report and results and create a script using the Rx pad on this page.
   Spring symptoms: tree allergies → treat using tree mix and add Alder or Cedar if needed
   Summer symptoms: grass allergies→ treat using grass mixes, add Bermuda single if needed Fall symptoms: weed allergies→ treat using common weed mix and add ragweed if positive
   Year round: dust or mold or pets→ mite or mold mix and pets as per symptoms
4. Total IgE can also help clarify a picture. A high total IgE may suggest high allergies, but a low does not always rule out allergies.
5. If a patient experiences allergy symptoms around a pet, you may add it even if they did not test positive. Allergy is a history based diagnosis and testing is confirmatory.

During the first dose observed visit you should:

-Set expectations and answer questions around SLIT

-Obtain consent to treat via SLIT

-Have patient administer their first drop

-Ensure no anaphylaxis or adverse reaction and observe for 30 minutes

How to set appropriate expectations:

-Drops will be taken every day at home and absorb under the tongue in roughly 1 minute

-Medication does not need to be refrigerated

-Most patients notice improvements in symptoms, or decreased need for antihistamines in 6-8 months

-If you have any allergy reactions while taking therapy please contact our clinic and stop therapy

-Please ensure you have an epi pen available and understand how to use it. The risk of a severe reaction however is very low.

Advent charges $750 for a 6 month subscription to allergy immunotherapy. It is not covered by insurance but patients can use their HSA and FSA card if available. This is still likely cheaper than the cost of copays for weekly allergy shots and easier than multiple visits with travel time.
